Twenty-two questions across the eight categories most often flagged in Appalachian Basin regulatory inspections — including Ohio-specific ODNR items. Answer Yes / Partial / No / N/A; get a color-coded risk rating and a written gap report.
Methodology & sources
Question selection draws on standard inspection items from:
- PA DEP — Title 25, Chap. 78a (unconventional), Chap. 78 (conventional), Chap. 127 (air)
- OH EPA / ODNR — OAC 3745 (air), ORC 1509 (oil & gas), OAC 1501:9 (ODNR DOGRM rules including Class II UIC, operator registration, and well-plugging assurance)
- WV DEP — CSR 35 (oil & gas), CSR 45 (air)
- 40 CFR — UIC, NSPS OOOOa/OOOOb (federal LDAR), SPCC (Part 112)
- OSHA — 29 CFR 1910 (general industry), 1926 (construction), 300 logs
- API standards — RP 12R1 (tank inspection), Spec 6A (wellhead), API 510 (pressure vessels)
Scoring: Yes = 1.0 point, Partial = 0.5, No = 0, N/A excluded from total. Final percentage = (sum / non-N/A count) × 100. Color thresholds: 90% Green (audit-ready), 70-89% Yellow (manageable gaps), 50-69% Orange (material gaps), <50% Red (significant exposure).
This is a self-assessment, not an audit. A real EHS program review involves on-site documentation inspection, interview with field personnel, and review of historical inspection records. The output is directional only.
